• Consent to Medical Care and Treatment of Minor Children (if students are under age 18)
• Alternative Accommodation (if students are under age 18)
• Airport Pick-Up Form (send later if you are not able to give information on housing application)
• Credit Card Authorization Form
• Agent Sponsorship Form
